Steps for New Patients or Expired Registrations

Onboarding and enrollment now have online registration. This can be done by calling a clinic near you and completing the link provided to you.

Once you are registered, you can meet with a provider.
How can our onboarding & enrollment team assist you?
Our online registration ensures all required documentation and information is collected before your appointment with a Provider and verifies your insurance. Our goal is to set you up for a successful experience when coming to an appointment.
An acute visit/emergency visit

If you are not an established patient, and the proper documents are not provided, you must pay the full cost.
