Onboarding and Enrollment ensure that patients have completed all necessary paperwork (registration form, medical history form) prior to their dental visit.

Documents needed for appointment:

• Medication list

• X-rays if they have gone to a previous dental clinic within 3 years.

• ​Patients applying for the slide will need: Proof of Income and Identification of each dependent/family member who lives with them.

 

O & E helps patients apply for our discount/slide if they do not have insurance. They also verify that their insurance is active and obtain benefits when they come in to avoid any billing issues.
