Can I combine my family accounts?
Yes, under one guarantor
Will I receive a statement?
Only if you have a balance due.
Can I pay my bill online?
Yes, you can create an account online through patient portal.
Will you bill my primary and secondary insurance?
All insurances will be billed.
Can I have insurance and also apply for Sliding Fee?
Yes, this is recommended.
Will front desk always ask for my name, address, phone # and copy of insurance card?
Yes, it is very important that we have current information at time of check in.
What forms of payment do you accept?
Credit cards, debit cards, cash, check, money order.
Do you offer financial assistance?
We have a sliding fee discount program and payment plans.
I have insurance but my statement doesn’t show it was billed?
Make sure front desk has a copy of your current insurance card.
Why am I asked for a copay at each visit?
You are responsible to pay copay at time of visit.
Insurance only paid part of my claim, why?
You may have a copay due, coinsurance or deductible.
What if my insurance is not in network?
You will need to pay at time of visit.
Reasons why your insurance might deny a claim:

Name does not match insurance card.

Incorrect subscriber name and DOB.

My insurance does not offer this type of coverage.

Denied for primary insurance.

Clinic/Provider not in network.
